GRADE SCHOOL DEPARTMENT
ASSESSMENT
Evaluation of pupil performance is
done in accordance with the following guidelines:
- A terminal skill per
subject and per level is identified and its acquisition made
requisite for movement to the next higher level.
- The components of this
skill is indicated in a learning continuum and programmed
according to the number of assessment periods in a year.The skills
in the learning continuum shall be the criteria for evaluation.
Mastery is always the goal and it is determined through such measures
as:

2.2 Testing Paper-Pencil tests

Formative test is a short test (usually 5-15
items) that checks mastery of a small segment of a skill.
·
Post test is given at the end of the unit to check
mastery of skills targeted for the period.
·
Mastery test is administered every three months to
check mastery of skills targeted for the period.
·
Performance test requires a pupil to perform/
demonstrate skills in P.E., work education, home economics, etc.
2.3 Requirements like book
reports, compositions, and other activities which further test mastery of the
skills

Rating
3.1 In determining the final assessment rating of the child,
three (3)
basic factors are
considered, namely: mastery of skills, based
primarily on the post and mastery
tests; recitation; and fulfillment of
specified
requirements for the period. Distribution of percentages for
the basic factors is as follows:

3.2 For academic subject, the final assessment rating is arrived at by
computing for the average of the final ratings in the
POST TEST/S,
MASTERY TEST,REQUIREMENTS, and RECITATION.
3.3 The following grading symbols are used in reporting pupil
performance:
E (Excellent) - Mastered all skills
prescribed for the period. Satisfied all requirements according to the
standard expectation for the growth level.
VS (Very Satisfactory) - Mastered 90% of the skills prescribed for the
period. Satisfied all requirements according to the standard
expectation for the growth level.
S+ (Satisfactory Plus) - Mastered 80% of the skills prescribed for the
peri6d. Satisfied all requirements according to the standard
expectation for the growth level.
S (Satisfactory) - Mastered 70% of the skills prescribed the period.
Will require referral service, several remediation sessions, etc.
MS (Moderately Satisfactory) - Mastered 60% of the skills prescribed
for the period. Will require remediation.
NI (Needs Improvement) - Mastered 50% of the skills prescribed for the
period. Will require referral service, several remediation sessions,
etc.
4.
Issuance of Report Card
Parents are officially informed of their child's performance through
the report card issued every assessment period. Such report card has to
be signed and returned three days after its issuance.
