2.4) Certified true copy of the current report card with the

second quarter grades. (Grade 2)
2.5) Recommendation from the Grade School Principal and Adviser
or
Guidance Counselor (Grade 2)
2.6) Photocopy of NSO birth certificate
2.7) Photocopy of baptismal certificate
2.8) Long brown envelope (Name of applicant in print and level
